Sasang Constitutional Phenotyping and the Evidence Gap in Precision Herbal Therapeutics: A Multi-Omics Roadmap

  • Abstract: Sasang Constitutional Medicine (SCM) classifies individuals into four constitutional types and proposes constitution-specific herbal treatment. However, its translation into precision therapeutics requires reproducible phenotyping and evidence that constitutional type predicts differential efficacy or toxicity. This Commentary evaluates current phenotypic, genomic, microbiome, pharmacological, and clinical evidence and proposes a multi-omics framework for validation. Existing studies indicate biological heterogeneity and formula-specific activity, but do not establish validated biomarkers or reproducible treatment-by-constitution interactions. Future studies should use chemically standardized herbal products, longitudinal multi-omics profiling, pharmacokinetic assessment, prespecified interaction analyses, and independent validation. SCM should therefore be regarded as a testable framework for precision herbal research rather than an established precision-treatment system.
